- Coordinate, monitor, plan, link and advocate to solve the service needs of the clients. - Mental health service providers. - Primary Care Physicians (HHA, Prescriptions, OTC medications, etc). - Social Security Administration. - Department of Children and Families (food stamps). - Housing opportunities available in the community. - Rehabilitation programs (individual and group therapy) - Community Action Agencies (LIHEAP). - Transportation. (STS, Handicap, On Demand) - Economic (food banks, cleaning products donations, etc)
